About the role:
The D2A service provides short-term, high-quality support to individuals who are well enough not to need an acute hospital bed but do need some extra support in their home.
Responsible for writing up the support plans following the initial visit and ensuring person-centred care is provided.
Motivating customers and building their confidence so they can achieve the goals they have set and return to independent living.
You will be supporting customers with tasks around the home and with personal care such as personal hygiene, toileting, and dressing.
Shifts will be a mixture of early and late shifts between 7 am and 10 pm and alternate weekends.
You will be required to be on-call on a roster basis which attracts an additional payment.

About you:
You will hold (or be working towards) a QCF Level 3 in Health and Social Care.
Previous experience in caring for vulnerable adults with a wide range of complex needs is essential.
Sensitive, caring, and passionate about delivering a first-class service to people within the local community.
A full driving licence and access to your own vehicle are required as you will need to be comfortable with lone working and travelling between customers.
You can be located in the Sedgemoor, Burnham, and Cheddar areas as travel time is included in your contracted hours.
Job Benefits:

Double time on bank holidays and an extra £1 per hour on weekends.
Mileage allowance of 45p per mile
Equivalent to 30 days of paid annual leave (inclusive of bank holidays), increasing to the equivalent of 33 after 5 years’ service (pro-rata)
