About the role:

  • The D2A service provides short-term, high-quality support to individuals who are well enough not to need an acute hospital bed but do need some extra support in their home.

  • Responsible for writing up the support plans following the initial visit and ensuring person-centred care is provided. 

  • Motivating customers and building their confidence so they can achieve the goals they have set and return to independent living. 

  • You will be supporting customers with tasks around the home and with personal care such as personal hygiene, toileting, and dressing. 

  • Shifts will be a mixture of early and late shifts between 7 am and 10 pm and alternate weekends.

  • You will be required to be on-call on a roster basis which attracts an additional payment. 

  • A £500 Welcome Bonus

 

About you:

  • You will hold (or be working towards) a QCF Level 3 in Health and Social Care. 

  • Previous experience in caring for vulnerable adults with a wide range of complex needs is essential. 

  • Sensitive, caring, and passionate about delivering a first-class service to people within the local community. 

  • A full driving licence and access to your own vehicle are required as you will need to be comfortable with lone working and travelling between customers.  

  • You can be located in the Sedgemoor, Burnham, and Cheddar areas as travel time is included in your contracted hours. 

 

Job Benefits:

  • £500 welcome bonus on successful completion of your probationary period. 

  • Double time on bank holidays and an extra £1 per hour on weekends.
